Should a swimmer be disqualified when, without some acceptable reason, they jump or dive into the water before or after taking up their starting position and before all the other swimmers were stationary?

Yes. The swimmer should be disqualified for the appropriate reason from the following “delaying the start”, “misconduct” or “willful disobedience” (SNSW SW 2.3.2 and SW 2.1.3).

What about when they fall into the water after taking up their starting position and all swimmers were stationary?

Yes. The swimmer should be disqualified for “starting before the starting signal”
